Office of the Secretary Federal Aviation Administration Federal Highway Administration Federal Motor Carrier Safety Administration Federal Railroad Administration Federal Transit Administration Maritime Administration National Highway Traffic Safety Administration If your employer is sending you for a test, they may have an authorization form already created for you. It is recommended to ask them for it before coming in. If you have your own Trucking Company, you may not have an authorization form and you may proceed with payment.
